In order to prepare a letter of recommendation, I need to have you to complete and sign a copy of the Reference Request and FERPA Release form and turn it in to me. This form is available at:

I would also find it helpful to read over the personal statement you are submitting. You could attach a copy and send it to me via email. If you want me to give you comments on it I can do that as well. Letting me read the personal statement is not mandatory, but it can be useful to me.
